We are seeking a highly organized and detail-oriented individual to join our team as a Home Health and Hospice Scheduler.
The ideal candidate will be responsible for coordinating and scheduling the delivery of home health and hospice services, ensuring that patient care is efficiently and effectively managed.
This role requires excellent communication skills, attention to detail, and the ability to work collaboratively with healthcare professionals, caregivers, and administrative staff.
Responsibilities:
Scheduling and Coordination:
Schedule home health and hospice services for patients based on physician orders and individual care plans.
Coordinate and organize the daily schedules of nurses, therapists, and other healthcare providers to ensure optimal patient care.
Communication:
Maintain regular communication with healthcare professionals, caregivers, and patients to confirm appointments and address any scheduling concerns.
Collaborate with the clinical team to ensure accurate and up-to-date information regarding patient needs and care plans.
Documentation and Record Keeping:
Maintain accurate and detailed records of scheduled appointments, patient preferences, and any changes in care plans.
Ensure compliance with regulatory requirements and organizational policies related to scheduling and documentation.
Problem Solving:
Anticipate and resolve scheduling conflicts or issues in a timely and efficient manner.
Collaborate with various departments to address any challenges that may arise during the scheduling process.
Quality Assurance:
Monitor and evaluate the effectiveness of scheduling processes, identifying areas for improvement and implementing solutions.
Contribute to quality assurance initiatives to enhance overall patient care and satisfaction.
Qualifications:
Education and
Experience:
High school diploma or equivalent; additional education or training in healthcare administration is a plus.
Previous experience in healthcare scheduling, preferably in home health or hospice settings.
Skills:
Strong organizational and multitasking abilities.
Excellent communication and interpersonal skills.
Proficient in using scheduling software and other relevant tools.
Knowledge of healthcare regulations and compliance standards.
Team Collaboration:
Ability to work collaboratively with a multidisciplinary team of healthcare professionals.
